Living Will Registry
Take care of entering your wishes in the US living will registry:
This non-profit corporation will store your Living Will/Advanced Directives in a central location and supply you with stickers for your driver's license, so that emergency room professionals nation-wide can have immediate faxed-back information regarding who is to make your medical decisions if you are unable, as well as just what level of intensity of health care you would wish, should you be unable to express your wishes at a critical time, due to medical incapacity. There is no charge for this service. The Registry does require that you send or fax them a notarized copy of your Living Will. They will ask you to reconfirm or update your information on an ongoing basis.
